Welcome to the 2017 FM Pop Warner Registration!

This online registration system will allow you to complete contact information, sign up for participation and submit payment. To complete registration through our secure site, please have your Visa, Discover, or MasterCard information available.  Following the completion of this registration, you will see a charge on your account. 

Please be sure to print the form packet (all forms are combined into one .pdf file) and submit them along with any necessary paperwork (birth certificate copy, etc.) to FM Pop Warner ASAP, so we can process your child's registration properly and avoid issues certifying per Pop Warner National rules.

Please see the table below for the Updated Age/Weight Matrix to help choose the correct team for your child.
The Registration System will choose the correct team(s) for your child's age/weight, but you will have the ability to choose if he/she falls into two different team levels.

Please note that the minimum and maximum weights have changed from previous years.  If you are unsure about which team to choose, pick the most appropriate team for your childs age/weight and then the Football Coordinator will place your child on the correct team.


Mandatory $100 fundraising fee (offset by selling fundraiser tickets) will be added at checkout. Family maximum fundraising fee is $150!

Required Forms and items for registration (all items are found in the registration packet below):

  • 2017 FM Pop Warner Registration Form
  • 2017 Pop Warner ID Card
  • 2017 Participant Contract and Consent Form
  • 2017 Medical Form - NO FAXED FORMS!  Must be original from Doctor.
  • 2017 FM Pop Warner Emergency Medical Auth Form
  • 2017 Participant Code of Conduct
  • 2017 Travel Policy

Other item's needed from each participant:

  • Copy of Birth Certificate
  • 2” x 2” photo
  • Copy of Report Card (both front and back are required). We cannot accept Report Cards until the end of the current school year.

Family Discount

If you are registering more than one player in your family this season, you will receive a discount on the second (and third, if applicable) child's registration fee.  In order for the registration system to recognize that your family is registering multiple players, please register all of your players at this time.  On the Review Page, there is a link near the bottom to "Register Another Player" - please click on this link to ensure that all of your family's players are registered within the same order.
