2018 FM Pop Warner Registration

Welcome to the 2018 FM Pop Warner Registration!

This online registration system will allow you to complete contact information, sign up for participation and submit payment. To complete registration through our secure site, please have your Visa, Discover, or MasterCard information available.  Following the completion of this registration, you will see a charge on your account. 

Please be sure to print the form packet (all forms are combined into one .pdf file) and submit them along with any necessary paperwork (birth certificate copy, etc.) to FM Pop Warner by August 1st, so we can process your child's registration properly and avoid issues certifying per Pop Warner National rules.

Please see the table below for the Updated Age/Weight Matrix to help choose the correct team for your child.
The Registration System will choose the correct team(s) for your child's age/weight, but you will have the ability to choose if he/she falls into two different team levels.

Please note that the minimum and maximum weights have changed from previous years.  If you are unsure about which team to choose, pick the most appropriate team for your childs age/weight and then the Football Coordinator will place your child on the correct team.

Registration Fee:

Tackle Football/Cheerleader       $125

Any 2 Athletes                             $225

Any 3 Athletes                             $300

Family Max Registration Fee       $350

Flag Football                                $80


Mandatory Fundraising Fee:

In addition to the registration fee (prices listed above), there is a mandatory $100 fundraising fee per athlete (offset by selling the green fundraiser tickets) that will be added at checkout (family maximum fundraising fee of $150).


Early Registration Discount:

Register by June 10th and receive a $10 discount off each athlete.

Required Documents for Registration

Please print and complete the form package below and return to FM Pop Warner by 8/1/2018. The registration packet can be mailed to the following address or hand delivered to Equipment Pickup at the Wellwood Equipment Shed.

Required Forms and items for registration (all items are found in the registration packet below):

  • Copy of Birth Certificate
  • 2" by 2" photo of athlete
  • 2018 Participant Contract and Consent Form
  • 2018 Physical Fitness & Medical History Form (physical must be completed by a physician in 2018)
  • 2017-2018 Report Card (please make a copy of the complete report card, including all quarters as well as the front and back of report card)
  • 2018 FM Registration Form
  • FM Pop Warner Emergency Medical Authorization Form
  • FM Pop Warner Participant Code of Conduct
  • FM Mandatory Volunteer Contract (work bond) NEW!

Family Discount

If you are registering more than one player in your family this season, you will receive a discount on the second (and third, if applicable) child's registration fee.  In order for the registration system to recognize that your family is registering multiple players, please register all of your players at this time.  On the Review Page, there is a link near the bottom to "Register Another Player" - please click on this link to ensure that all of your family's players are registered within the same order.
